In-Home Traveler Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Massachusetts.

📋 Description

• Deliver in-home health evaluations at the residences of members.

• Establish trusted relationships and contribute to improved health outcomes.

• Conduct visits lasting approximately 45 minutes.

• Review medications and medical histories of members.

• Execute physical evaluations.

• Carry out additional diagnostic tests as ordered, such as diabetic eye exams and spirometry.

• Utilize an iPad and clinical workflow for each evaluation.

• Conduct virtual visits when necessary.

• Provide diagnostic and preventive services, including spirometry, DEE, and bone density screenings.

• Travel locally and within licensed states, covering specified areas and all required counties.

• Function as a clinician colleague for Signify Health.

⛳️ Requirements

• Qualification as a Nurse Practitioner or Physician Assistant.

• Possession of an active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Ability to obtain and maintain multi-state licenses in the applicable coverage area, as necessary.

• Capacity to cover all counties within the state and adapt to other business needs as required.

• Ability to safely perform duties in homes with household pets, secondhand smoke, and potential airborne allergens or irritants.

• Flexibility to travel locally and within licensed states.

• Willingness to travel up to 100% of the time, which may include overnight hotel stays, car rentals, and/or flights.
