In-Home Traveler Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Conduct health evaluations in the homes of members.

• Engage with individuals to help close care gaps.

• Travel mainly within a specified coverage area, potentially up to 100% of the time.

• Perform visits lasting approximately 45 minutes.

• Review medications and medical histories.

• Conduct physical examinations.

• Execute additional diagnostic tests as requested by the health plan, including diabetic eye exams and spirometry.

• Utilize an iPad and clinical workflow for each evaluation.

• Conduct virtual visits when necessary.

• Provide Signify Health services, encompassing Diagnostic & Preventive Services like spirometry, diabetic eye exams, and bone density screenings.

• Establish trusted relationships and offer personalized health evaluations.

• Visit health plan members across the nation and connect them with appropriate care.

⛳️ Requirements

• Nurse Practitioner or Physician Assistant qualification.

• Active, unrestricted licenses in the relevant coverage areas.

• Board certification is mandatory.

• Capability to obtain and maintain multi-state licenses as required.

• Ability to cover all counties within the state and adapt to other business needs.

• Comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Willingness and ability to travel locally and within licensed states, potentially up to 100% of the time.

• Ability to accommodate overnight hotel stays, car rentals, and/or flights as needed.

• Competence in conducting virtual visits when required.
