This is a fully remote position, open to applicants in Colorado.

📋 Description

• Conduct in-home health assessments for members at their residences.

• Foster trustworthy relationships to enhance health outcomes.

• Execute visits lasting approximately 45 minutes, which include reviewing medication and medical history as well as performing physical evaluations.

• Carry out additional diagnostic tests as prescribed by the member's health plan, including diabetic eye exams, spirometry, and bone density screenings.

• Utilize an iPad and a user-friendly clinical workflow for each evaluation.

• Facilitate virtual appointments as necessary.

• Deliver Signify Health services, encompassing Diagnostic & Preventive Services products.

• Travel locally and within licensed states, ensuring coverage across all counties and other regions as required.

• Engage with individuals nationwide to connect them with suitable care.

⛳️ Requirements

• Qualification as a Nurse Practitioner or Physician Assistant.

• Possession of an active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Capability to secure and maintain multi-state licenses in the coverage area, as needed.

• Availability to cover all counties within the state and flexibility to address other business needs.

• Comfort and safety in performing duties in homes that may have household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Willingness and ability to travel locally and within licensed states, potentially up to 100% of the time.

• Availability for overnight hotel stays, car rentals, and/or flights as required by the coverage area.

• Expected weekly hours: 40.
