In-Home Traveler, NP/PA

This is a fully remote position, open to applicants in Pennsylvania, +2 more states.

📋 Description

• Conduct in-home health assessments for members in their residences.

• Perform visits lasting around 45 minutes each.

• Interact directly with members to address their health inquiries.

• Review patients' medications and medical histories.

• Execute physical evaluations.

• Carry out additional diagnostic tests as directed by the health plan, such as diabetic eye examinations and spirometry tests.

• Utilize an iPad and follow clinical workflows for each assessment.

• Offer virtual consultations as necessary.

• Provide Diagnostic & Preventive Services, which includes spirometry, diabetic eye tests, and bone density screenings.

• Travel within the defined coverage area, both locally and across licensed states.

• Engage with individuals nationwide to link them with suitable care.

• Establish trustworthy relationships to enhance health outcomes.

⛳️ Requirements

• Qualification as a Nurse Practitioner or Physician Assistant.

• Possess an active, unrestricted license(s) in relevant coverage areas.

• Board certification is mandatory.

• Capability to acquire and maintain multi-state licenses in the required areas.

• Flexibility to cover all counties within the state and adapt to other business needs.

• Comfortably and safely perform responsibilities in homes with pets, secondhand smoke, and other potential airborne allergens or irritants.

• Willingness to travel locally and within licensed states.

• Availability for travel up to 100% of the time, which may include overnight stays, car rentals, and/or flights.

• Commitment to working 40 hours per week.
