In-Home Travel Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Conduct In-Home Health Evaluations within the residences of members.

• Foster engagement and establish trusted relationships to enhance health outcomes.

• Review patient medication and medical history.

• Execute physical evaluations.

• Perform additional diagnostic tests as ordered by the health plan, including diabetic eye exams, spirometry, and bone density screenings.

• Utilize an iPad and clinical workflow for each evaluation conducted.

• Travel nationwide to connect individuals with appropriate care.

• Conduct virtual visits when necessary.

• Offer Signify Health services, encompassing Diagnostic & Preventive Services products.

• Engage in local travel and within licensed states, which may involve overnight hotel stays, car rentals, and/or flights.

⛳️ Requirements

• Nurse Practitioner or Physician Assistant qualification.

• Active and unrestricted license(s) in the relevant coverage areas.

• Board certification is mandatory.

• Capable of obtaining and maintaining multi-state licenses in the required coverage areas.

• Willingness to cover all counties within the state and adapt to other business needs.

• Comfortable and safe performing duties in homes that may have household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Ability to travel locally and within licensed states, potentially up to 100% travel.

• Multiple state licenses as required by business demands.
