In-Home Travel Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Connecticut.

📋 Description

• Conduct health evaluations in the homes of members.

• Travel extensively, potentially up to 100% of the time, within a designated coverage area.

• Carry out visits that last approximately 45 minutes.

• Interact individually with members and address their health inquiries.

• Review patients’ medications and medical histories.

• Perform physical assessments.

• Execute additional diagnostic tests as directed by the health plan, such as diabetic eye examinations and spirometry tests.

• Utilize an iPad and clinical workflow for each evaluation.

• Conduct virtual consultations as required.

• Provide Diagnostic & Preventive Services, including spirometry, diabetic eye tests, and bone density screenings.

• Establish trustworthy relationships to enhance health outcomes.

• Travel may require overnight stays in hotels, rental cars, and/or flights.

• Collaborate as a clinician colleague, fulfilling the specified responsibilities.

⛳️ Requirements

• Active, unrestricted licenses in the relevant coverage areas.

• Board certification (mandatory).

• Capability to obtain and maintain multi-state licenses as necessary.

• Ability to cover all counties within the state and adapt to other business needs as required.

• Must be able to safely and comfortably perform duties in environments that may include household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Willingness to travel locally and within licensed states.

• Qualification as a Nurse Practitioner or Physician Assistant.

• Availability for full-time work, with an anticipated 40 hours per week.
