In-Home Physician

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Deliver part-time in-home health assessments to members of health plans.

• Perform visits lasting approximately 45 minutes, which include reviewing medication and medical history as well as conducting physical evaluations.

• Execute additional diagnostic tests as prescribed by the individual’s health plan, such as diabetic eye exams and spirometry.

• Utilize an iPad and follow clinical workflows for each assessment.

• Establish trusted relationships to enhance health outcomes.

• Offer referrals to health plans and promote the involvement of family members.

• Ensure that primary care physicians are informed about patients’ health status and quality of life.

• Conduct virtual appointments as required.

• Provide Signify Health diagnostic and preventive services, including spirometry, diabetic eye exams, and bone density screenings.

• Travel locally and within licensed states as necessary.

⛳️ Requirements

• Possess an active, unrestricted license(s) in relevant coverage areas.

• Multi-state licenses are preferred.

• Must be capable of safely and comfortably performing duties in homes that may have household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Ability to travel locally and within licensed states as required.

• Capability to conduct virtual visits as needed.

• Must hold an MD or DO qualification.
