In-Home Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Washington.

📋 Description

• Conduct in-home health assessments within members’ residences.

• Foster engagement with members to establish trustworthy relationships.

• Review patients' medication and medical histories.

• Execute physical evaluations.

• Carry out additional diagnostic tests as prescribed by the health plan, including diabetic eye exams, spirometry, and bone density screenings.

• Utilize an iPad and clinical workflow for each evaluation.

• Travel nationwide to connect individuals with suitable care.

• Deliver Signify Health services, encompassing Diagnostic & Preventive Services products.

• Conduct virtual visits as necessary.

• Travel locally and within licensed states, which may involve overnight hotel accommodations, car rentals, and/or flights.

⛳️ Requirements

• Qualification as a Nurse Practitioner or Physician Assistant.

• Possession of active, unrestricted license(s) in applicable coverage areas.

• Board certification is mandatory.

• Capability to acquire and sustain multi-state licenses within the coverage area, as required.

• Ability to cover all counties within the state and adapt to other business needs.

• Willingness to travel locally and across licensed states.

• Flexibility to travel up to 100% of the time within a defined coverage area.

• Competence to perform duties safely and comfortably in homes that may have household pets, secondhand smoke, and potential airborne allergens or irritants.

• Proficiency in conducting virtual visits as required.
