In-Home Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Conduct in-home health evaluations for members at their residences.

• Foster engagement with members to build trustworthy relationships.

• Carry out visits lasting approximately 45 minutes.

• Assess medications and review medical histories.

• Execute physical evaluations.

• Administer additional diagnostic tests as ordered, such as diabetic eye exams, spirometry, and bone density screenings.

• Utilize an iPad and follow clinical workflows for every evaluation.

• Perform virtual visits as required.

• Deliver Signify Health services, including Diagnostic & Preventive Services products.

• Travel locally and within licensed states, covering all counties in the state and other areas as necessary.

• Assist in identifying connections to suitable care and enhancing health outcomes.

⛳️ Requirements

• Possess an active, unrestricted license(s) in the relevant coverage area(s).

• Hold board certification (mandatory).

• Capable of obtaining and maintaining multi-state licenses in the coverage area, as required.

• Able to cover all counties within the state and adapt to other business needs.

• Must be comfortable and capable of performing duties in homes that may have household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Qualification as a Nurse Practitioner or Physician Assistant.

• Willingness to travel locally and within licensed states.

• Ability to travel up to 100% of the time, which may include overnight hotel stays, car rentals, and/or flights.
