In-Home Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Iowa.

📋 Description

• Conduct in-home health assessments at members’ residences.

• Travel up to 100% of the time within a designated coverage area, which may involve overnight hotel stays, car rentals, and/or flights.

• Establish trustworthy relationships to enhance health outcomes.

• Perform visits that last approximately 45 minutes.

• Review medications and assess medical history.

• Execute physical evaluations.

• Carry out additional diagnostic tests as prescribed by the health plan, including diabetic eye exams and spirometry tests.

• Utilize an iPad and clinical workflow during each evaluation.

• Conduct virtual visits when necessary.

• Deliver Signify Health services, encompassing Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screenings.

• Serve all counties in Iowa and adapt to other areas based on business needs.

• Collaborate as a clinician colleague with Signify Health.

⛳️ Requirements

• Possess an active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Must be capable of obtaining and maintaining multi-state licenses in the coverage area, as needed.

• Must be able to cover all counties within the state and adapt to other areas as required by the business.

• Ability to perform duties comfortably and safely in homes that may have pets, secondhand smoke, and potential airborne allergens or irritants.

• Qualification as either a Nurse Practitioner or Physician Assistant.

• Willingness and capacity to travel locally and within licensed states, including up to 100% travel.

• Capability to conduct virtual visits as necessary.

• Proficient in using an iPad and clinical workflow.

• Expected availability of 40 hours per week.
