In-Home Nurse Practitioner or Physician Assistant, Full Time

This is a fully remote position, open to applicants in Maine.

📋 Description

• Conduct in-home health evaluations at the residences of members.

• Interact personally with health plan members to address their health inquiries.

• Assess medications and review medical histories.

• Execute physical evaluations.

• Perform additional diagnostic tests as needed, such as diabetic eye exams, spirometry, and bone density screenings.

• Utilize an iPad and clinical workflow for each assessment.

• Travel nationwide to connect individuals with suitable care.

• Offer Signify Health services, which include Diagnostic & Preventive Services products.

• Conduct virtual visits as necessary.

• Travel locally and within licensed states, accommodating areas based on business requirements.

• Establish trusting relationships to enhance health outcomes.

⛳️ Requirements

• Possess an active, unrestricted license in the relevant coverage areas.

• Board certification is mandatory.

• Capable of obtaining and maintaining multi-state licenses as required.

• Must be able to cover all counties within the state and adjust to other business needs as necessary.

• Qualifications as a Nurse Practitioner or Physician Assistant.

• Ability to perform duties safely in homes that may have pets, secondhand smoke, and airborne allergens or irritants.

• Willingness to travel locally and within licensed states.

• Capability to travel up to 100% of the time within a specified coverage area.

• Prepared to accommodate overnight hotel stays, car rentals, and/or flights as needed.

• Ability to conduct virtual visits when required.
