In-Home Nurse Practitioner, Full Time

This is a fully remote position, open to applicants in Rhode Island.

📋 Description

• Conduct health evaluations within the homes of members.

• Connect with individuals to help address care gaps.

• Carry out visits lasting approximately 45 minutes.

• Review patients' medications and medical histories.

• Perform thorough physical assessments.

• Administer additional diagnostic tests as requested, such as diabetic eye exams and spirometry.

• Utilize an iPad and clinical workflow for each assessment.

• Establish trusted relationships to enhance health outcomes.

• Travel locally and within licensed states.

• Conduct virtual appointments as necessary.

• Deliver Signify Health services, which include diagnostic and preventive services like spirometry, diabetic eye exams, and bone density screenings.

• Visit individuals nationwide to connect them with suitable care.

⛳️ Requirements

• Possess an active, unrestricted license(s) in relevant coverage area(s).

• Obtain board certification (mandatory).

• Capable of acquiring and maintaining multi-state licenses as required for coverage areas.

• Able to cover all counties within the state and adaptable to other business needs.

• Primarily travel up to 100% of the time within a designated coverage area.

• Competent to perform duties in homes with household pets, secondhand smoke, and potential airborne allergens or irritants.

• Qualification as a Nurse Practitioner or Physician Assistant.

• Anticipate working 40 hours per week.
