In-Home Nurse Practitioner, Full Time

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Conduct health evaluations in the homes of members.

• Interact with individuals to help close care gaps.

• Perform visits lasting around 45 minutes, which include reviewing medication and medical history as well as a physical assessment.

• Execute additional diagnostic tests as ordered by the health plan, such as diabetic eye exams, spirometry, and bone density screenings.

• Utilize an iPad and follow a clinical workflow for every evaluation.

• Carry out virtual visits when necessary.

• Deliver Signify Health services, including products related to Diagnostic & Preventive Services.

• Establish trustworthy relationships and contribute to improved health outcomes.

• Travel nationwide to connect individuals with suitable healthcare options.

• Travel locally and within licensed states, covering designated counties and areas as per business requirements.

⛳️ Requirements

• Nurse Practitioner or Physician Assistant qualification.

• Possess an active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Capable of obtaining and maintaining multi-state licenses in the coverage area, as needed.

• Able to cover all counties within the state and adapt to other business needs.

• Comfortable and safe performing duties in homes with pets, secondhand smoke, and potential airborne allergens or irritants.

• Must be able to travel locally and within licensed states.

• Willing to travel up to 100% of the time, which may include overnight stays in hotels, car rentals, and/or flights.

• Expected weekly working hours: 40.
