Implementation Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee the complete ECC onboarding process for new practices, from initiation to initial outreach and the subsequent post-launch review.

• Manage timelines for multiple simultaneous implementations and proactively escalate any delays.

• Coordinate both implementation and clinical workstreams, which include patient data intake and validation, materials and forms, outreach messaging, after-hours support, billing processes, and system access.

• Maintain updated contact maps for practices, including leadership, project champions, clinical leads, data/EHR, and billing contacts.

• Develop agendas and prepare, refine, and version meeting materials and presentations.

• Facilitate meetings with the Program Director or independently, steering discussions towards decisions and assigning ownership.

• Document and distribute follow-up actions, ensuring the completion of post-meeting deliverables and forms.

• Prepare regular internal and practice-facing status updates for leadership review.

• Act as the primary daily contact for partner practice leadership, administration, clinical leads, billing, and EHR/data contacts.

• Set expectations regarding timelines, data needs, and responsibilities on the practice side.

• Identify and communicate practice-side risks and obstacles to the Program Director.

• Maintain a comprehensive record of deadlines, commitments, and outstanding items, ensuring they are resolved.

• Collaborate with the Clinical Program Director on staffing readiness, training schedules, and launch pacing.

• Work with Finance, Legal and Compliance, Data, People Operations, and Market Operations to facilitate work processes.

• Oversee vendor and platform coordination for both launch and ongoing operations.

• Identify recurring process issues and suggest sustainable solutions such as checklists, templates, and trackers.

• Perform additional duties as assigned that align with the role.

⛳️ Requirements

• Minimum of 5 years of experience in implementation, program or project management, client success, or practice/provider operations, preferably in healthcare services, value-based care, or healthcare technology.

• Proven ability to manage multi-stakeholder implementations within a set timeline involving external customers or partners.

• Exceptional written and verbal communication skills, including proficiency in creating polished presentations and facilitating meetings with clinicians and executives.

• Strong organizational skills and commitment to follow-through; capable of working independently in ambiguous situations.

• Advanced proficiency in the MS Office Suite, including Outlook, Teams, SharePoint, Excel, and PowerPoint.

• Willingness to travel occasionally for practice kickoffs and on-site meetings.

• Home internet connection must support a minimum download speed of 25 Mbps and upload speed of 10 Mbps; Cable, Fiber, or DSL connections directly wired to the internet device are preferred.

• Employees must operate from a dedicated workspace free from ongoing interruptions to protect member PHI / HIPAA information.

• Preferred: Experience in nephrology, chronic care management, or other CMS care management programs.

• Preferred: Understanding of provider practice operations, EHR data workflows, and secure patient data transfer processes.

• Preferred: Experience in managing relationships with vendors or platform partners.

• Preferred: Ability to quickly learn care management platforms and reporting tools.
