This is a fully remote position, open to applicants in North Carolina.

• Engage with Members: Evaluate members' needs to create Person-Centered Care Plans.

• Collaborative Leadership: Lead interdisciplinary team meetings to address the member's needs.

• Transition Specialist: Support members through life transitions and link them to essential services.

• Empowerment: Inform members and their families about their rights and connect them to appropriate services.

⛳️ Requirements

• You've obtained your degree and successfully applied it in your career!

• Bachelor's degree in a human services discipline such as psychology, social work, nursing, or another relevant field, with a minimum of 2 years of full-time experience working with individuals with Intellectual and Developmental Disabilities.

• At least 2 years of experience with individuals who have significant Long-Term Services and Supports (LTSS) needs due to their disability in a community care environment.

• OR Bachelor's degree in a field other than human services and a minimum of 4 years of full-time experience working with individuals with Intellectual and Developmental Disabilities.

• OR Master's degree with at least 1 year of full-time experience working with individuals with Intellectual and Developmental Disabilities.

• Willingness to travel regularly as required to fulfill job responsibilities.
