Hospice Triage RN

This is a fully remote position, open to applicants in United States.

📋 Description

• Provide after-hours triage, clinical assessment, and interventions via phone.

• Assist patients and their families with symptom management and end-of-life care needs.

• Coordinate care through collaboration with physicians, facilities, and internal teams.

• Organize visits and coordinate services with vendors as necessary.

• Offer clear, compassionate education and support to families.

• Accurately record all interactions, care plans, and outcomes in the EMR.

• Review team documentation for accuracy and compliance.

• Collaborate with Telecare staff to ensure positive patient outcomes.

⛳️ Requirements

• Active RN license (with no current or pending restrictions).

• Experience in hospice care.

• Familiarity with Home Care Home Base (HCHB).

• Strong clinical judgment and critical thinking skills.

• Excellent communication skills and proficiency in phone-based patient care.

• Ability to multitask effectively in a fast-paced, remote environment.

• Strong customer service abilities and conflict resolution skills.
