
Hospice Journey Counselor

This is a fully remote position, open to applicants in United States.
• Oversee the patient census and referrals for the Journey program within the designated area.
• Assess patient charts to evaluate eligibility and suitability for Hospice services.
• Initiate contact with new Journey referrals via phone and perform follow-up communications.
• Organize and execute home visits for patients, their families, and caregivers.
• Engage in discussions regarding end-of-life planning, Hospice philosophy, goals, eligibility, and benefits.
• Maintain accurate patient records and document all visits and communications.
• Foster strong relationships and effective communication with Clinicians.
• Collaborate and communicate with the Palliative care team and Nurse Practitioner.
• Participate in monthly Interdisciplinary team meetings.
• Achieve weekly goals for patient visits and phone calls, along with monthly hospice transition objectives.
• Support Hospice and/or Home Health teams with transitions to Hospice, which includes visits, calls, paperwork, and other related tasks.
• Carry out additional responsibilities as assigned.
• High school diploma or GED.
• Previous experience in home healthcare or a related role within the healthcare sector is preferred.
• Familiarity with the hospice philosophy is preferred.
• Must possess and uphold a professional license, certificate, or registration, as required.
• Completion of a drug screening is mandatory prior to employment.
• Must be legally authorized to work in the United States.
• Capability to perform essential job functions and meet specified physical demands, including frequently lifting or moving up to 20 pounds.
• Willingness and ability to travel by car and work in varying weather conditions.
• Full-time employment.
• 40 working hours each week.
