Home Infusion RN – Independent Contractor

This is a fully remote position, open to applicants in New York.

📋 Description

• Perform in-home infusion therapy visits in accordance with the established care plan.

• Prepare and document clinical/nursing notes as well as patient interventions in both clinical and non-clinical records.

• Serve as a clinical resource and collaborate effectively with pharmacy departments.

• Keep updated on drug side effects and potential interactions.

• Provide both telephone and in-person education regarding therapy protocols, drug side effects, and home infusion procedures.

• Ensure that patients comprehend their therapy plans, medications, and adherence requirements.

• Act as a liaison between patients, their families, and the pharmacy.

• Conduct thorough initial and periodic head-to-toe assessments of patients.

• Initiate preventative and rehabilitative care as necessary.

• Assess patient needs utilizing assessment data and feedback from the agency team, physician, patient, and family.

• Manage expectations of patients and families concerning services, outcomes, and discharge goals.

• Establish primary and secondary diagnoses based on assessments and the focus of home health care.

• Create care plans that include skilled interventions, medical supplies, and required services.

• Evaluate patient progress with team members and adjust care plans as necessary.

• Carry out additional duties as assigned.

• Operate under the supervision of the Director of Nursing.

⛳️ Requirements

• Graduate from an accredited nursing school.

• Current RN state license is required.

• A minimum of 2 years of experience in home infusion and infusion centers.

• At least 2 years of experience with IV Immunoglobulins and IV Biologics.

• CRNI certification is preferred.

• Prior experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients is preferred.

• Strong interpersonal, time management, and organizational skills are essential.

• Excellent written and verbal communication skills are required.

• Ability to maintain precise and accurate records consistently.

• Capability to direct information to the appropriate healthcare provider.

• Proficient computer skills, including word processing, internet usage, and email communication.

• Current CPR and BLS certification from the Red Cross or AHA.

• Valid driver’s license is necessary.

• Reliable transportation is required.

• Automobile insurance must be provided.

• Ability to sit or stand, move inside and outside patients’ homes, operate a computer and office machinery, communicate frequently via telephone and text, and lift up to 20 pounds.
