This is a fully remote position, open to applicants in Massachusetts.
• Conduct in-home infusion therapy visits in accordance with the established care plan
• Prepare clinical and nursing documentation, recording patient interventions in both clinical and non-clinical records
• Act as a clinical resource while collaborating with pharmacy departments
• Stay informed about drug side effects and interactions
• Provide education to patients via telephone and in-person regarding therapy protocols, side effects of medications, and home infusion procedures
• Ensure that patients comprehend their therapy plans, medications, and adherence requirements
• Serve as a liaison between patients, their families, and the pharmacy
• Conduct thorough initial and ongoing head-to-toe patient assessments
• Initiate preventative and rehabilitative care
• Assess patient needs using health evaluation data along with input from the care team, physician, patient, and family
• Manage expectations of patients and families concerning services, outcomes, and discharge objectives
• Establish primary and secondary diagnoses based on patient evaluations and the focus of home health care
• Develop care plans that include skilled interventions, necessary medical supplies, and services
• Evaluate patient progress and update care plans in collaboration with team members
• Work under the supervision of the Director of Nursing
• Perform additional duties as assigned
• Graduate from an accredited nursing program
• Valid RN state license
• Over 2 years of experience in home infusion and infusion centers
• More than 2 years of experience with IV Immunoglobulins and IV Biologics
• CRNI certification is preferred
• Preferred experience in peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients
• Strong interpersonal, time management, and organizational abilities
• Excellent written and verbal communication skills
• Ability to maintain consistently accurate documentation
• Capability to direct information to the appropriate healthcare provider
• Proficient computer skills, including word processing and effective use of the internet and email
• Current CPR and BLS certification from the Red Cross or AHA
• Valid driver’s license, dependable transportation, and automobile insurance
• Ability to lift up to 20 pounds on occasion
• Ability to maintain constant communication with patients and the team via telephone and texting
• Flexible working hours
