Home Health Registered Nurse

This is a fully remote position, open to applicants in New York.

📋 Description

• Deliver proficient nursing care to patients in their residences, which includes administering medications, performing wound care, and conducting patient evaluations.

• Collaborate with physicians and other healthcare professionals to develop and implement tailored care plans.

• Monitor and assess patients' conditions, reporting any changes as necessary.

• Provide education to patients and their families regarding disease management, medication usage, and treatment alternatives.

• Keep accurate records of patient care and coordinate effectively with healthcare professionals.

• Communicate patient care progress and condition updates to the patient's physician and Clinical Manager.

• Supervise Home Health Aides and ensure the proper completion of documentation.

• Report directly to the Clinical Manager.

• Offer coverage in the Liverpool and North Syracuse areas.

⛳️ Requirements

• Experience in med surg, ICU, ER, or acute care settings.

• Valid and unrestricted Registered Nurse licensure.

• Current CPR certification.

• Proven experience in collaborating with a team of healthcare professionals.

• Possession of a valid driver's license.

• Auto insurance coverage.

• Reliable means of transportation.

• Must maintain personal vehicle liability insurance in compliance with state minimum requirements or specified Humana limits.

• One year of nursing experience is preferred.

• Experience in Home Health is preferred.
