Home Health Psych Registered Nurse – Per Diem

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Deliver professional nursing care to patients in their residences, which includes administering medications, performing wound care, and executing patient assessments.

• Collaborate with physicians and other healthcare professionals to create and implement personalized care plans.

• Observe and assess patients' conditions, reporting any changes as necessary.

• Provide education to patients and their families regarding disease management, medication, and treatment alternatives.

• Keep precise records of patient care and liaise with healthcare professionals.

• Communicate progress in patient care and condition to the patient's physician and Clinical Manager.

• Supervise Home Health Aides and ensure proper documentation is completed.

⛳️ Requirements

• Experience in Med surg, ICU, ER, or acute care settings.

• Possession of current and unrestricted Registered Nurse licensure.

• Up-to-date CPR certification.

• Proven track record of working collaboratively with a team of healthcare professionals.

• Valid driver's license, auto insurance, and dependable transportation.

• Preferred one year of nursing experience.

• Home Health experience is preferred.

• Required TB screening for this patient-facing position.

• Valid state driver's license along with personal vehicle liability insurance.
