Home Health Licensed Practical Nurse

This is a fully remote position, open to applicants in New York.

📋 Description

• Deliver personalized, one-on-one skilled nursing care to patients in their homes under the supervision of an RN.

• Execute individualized care plans in collaboration with patients, families, and the healthcare team.

• Provide education to patients and families regarding disease management, treatment alternatives, and self-care practices.

• Keep precise patient care records, documenting visits, interventions, and outcomes.

• Communicate swiftly with patients, physicians, clinical managers, and healthcare team members concerning needs and changes in status.

• Engage in care coordination and discharge planning.

• Assist patients in regaining independence and recovering in their home environment.

⛳️ Requirements

• Valid nursing license in the state of practice.

• Current driver's license.

• Auto insurance coverage.

• Dependable transportation.

• Up-to-date CPR certification.

• Minimum of two years of experience as an LPN/LVN in a clinical environment.

• Experience in a Home Health or Hospice setting is preferred.

• Must maintain personal vehicle liability insurance as per state minimum requirements or specified higher limits.
