This is a fully remote position, open to applicants in Philippines, +1 more country.
β’ Receive and handle new referrals from hospitals, physicians, and community sources.
β’ Verify patient eligibility, insurance coverage, and documentation necessary for Medicare certification.
β’ Obtain and assess physician orders prior to initiating care.
β’ Coordinate prompt start-of-care visits in alignment with Medicare Conditions of Participation.
β’ Serve as a liaison among patients, families, physicians, clinicians, and community resources.
β’ Establish, update, and oversee patient care plans in accordance with CMS guidelines and physician orders.
β’ Monitor visit frequencies, track missed visits, and evaluate outcomes.
β’ Communicate any changes in patient condition or physician orders to appropriate team members.
β’ Accurately enter patient data into the Electronic Health Record system.
β’ Maintain Medicare documentation, including OASIS assessments, care plans, and visit notes.
β’ Engage in Quality Assurance and Performance Improvement initiatives.
β’ Review clinical documentation prior to billing submission.
β’ Assist with internal and external audits as well as regulatory surveys.
β’ Schedule RNs, PTs, OTs, STs, and HHAs according to patient needs, clinician expertise, and visit requirements.
β’ Communicate updates regarding schedules, missed visits, and urgent patient needs.
β’ Collaborate with clinical leadership to ensure adequate staffing coverage.
β’ Provide exceptional service to patients, families, referral sources, and staff.
β’ Address and resolve concerns raised by patients or caregivers.
β’ Foster a positive agency image within the community and among referral partners.
β’ Must hold a US Registered Nurse license in New York.
β’ A minimum of 2 years of experience in home health care, case management, or clinical coordination.
β’ Familiarity with Medicare Conditions of Participation.
β’ Understanding of OASIS documentation.
β’ Knowledge of HIPAA compliance.
β’ Strong organizational and time-management capabilities.
β’ Ability to juggle multiple priorities effectively.
β’ Excellent verbal and written communication skills.
β’ Proficient in Home Health EHR software (e.g., Net Smart, Homecare Homebase, MatrixCare, or similar).
β’ Detail-oriented with a focus on ensuring accuracy in documentation.
β’ Empathetic, patient-centered, and dedicated to delivering quality care.
β’ Reliable computer.
β’ Noise-canceling headset.
β’ Second monitor.
β’ Stable internet connection.
β’ Backup internet connection.
β’ Availability to work US Eastern Standard Time (EST), from 9 am to 6 pm.
