This is a fully remote position, open to applicants in United States.
• Reconcile logs from the Emergency Department, Urgent Care, and/or Observation with patient charts to facilitate coding and billing.
• Generate a minimum of 85-100 charts daily by manually pulling or processing stray and deficient charts.
• Obtain logs and charts through EMR, SFTP, DFS, scanning, and other feed sources.
• Format logs and charts into the required Excel or alternative formats.
• Analyze charts for necessary documents, ensuring provider documentation is complete and that the date of service, account number, and MRN information match.
• Submit batches of charts via Chart Receiver and verify receipt in Kofax, ensuring proper properties, legibility, orientation, page limits, and removal of unnecessary pages.
• Identify and categorize deficient and non-billable charts using approved reasons.
• Investigate and resolve outstanding strays, EDRC exceptions, and deficient charts at least bi-weekly or as reports are returned.
• Acquire, reconcile, analyze, and submit inpatient logs and charts monthly via Kofax by the 10th of each month.
• Submit logs and charts within 3-4 days of the date of service or according to facility-specific deadlines using Chart Portal and Chart Receiver.
• Support offshore audits, non-bill audits, and various other projects.
• Communicate with providers regarding incomplete charts through in-person interactions, EMR, phone, My SCP App, or email.
• Meet departmental goals for chart submission lag-time and monthly deadlines.
• Report daily production metrics using JotForm.
• Uphold HIPAA and SCP Health compliance; report any concerns to the Manager or Compliance Team.
• Collect SCP Health mail at the facility and forward it to the Dallas office weekly if HI-C is onsite.
• Act as an onsite SCP Health representative for providers and hospital staff.
• Perform additional assigned tasks.
• High school diploma.
• 0-2 years of experience in Medical Records.
• Preferred experience in Medical Records, HIM, Healthcare Clinic, or Physician Office, though not required.
• Detail-oriented with the ability to organize tasks and prioritize effectively.
• Basic proficiency in Microsoft Office, including Excel, Word, and Outlook.
• Professional demeanor and appearance to interact with hospital staff and providers.
• Ability to adhere to SCP Health Compliance policies and HIPAA guidelines.
• Capability to work cross-trained at one or two additional sites.
• Ability to work onsite when applicable and comply with hospital policies.
