This is a fully remote position, open to applicants in Pennsylvania.
• Assist members in achieving their health objectives and fulfilling the company Wellness program goals through virtual health coaching sessions and diabetes education.
• Participate in both the Diabetes and Lifestyle coaching teams, with time allocated based on seasonal demands and client requirements.
• Support members in establishing and accomplishing their health goals and objectives.
• Tailor coaching and educational approaches to meet members’ physical, psychological, cultural, and age-specific needs.
• Exhibit proficient customer service skills.
• Familiarize yourself with each company’s wellness programs and help members stay on track to achieve program goals.
• Adhere to established program policies and procedures to ensure consistent coaching practices and maintain program quality.
• Guide members in areas such as sleep, physical activity, nutrition, stress management, weight control, tobacco cessation, and enhancing biometric measures.
• Deliver health coaching and diabetes education for members diagnosed with prediabetes, type 1, and type 2 diabetes.
• Analyze member-reported health information, including glucose levels, to identify trends and patterns.
• Assist members in interpreting their health data and promote appropriate follow-up with their personal healthcare providers.
• Provide education and coaching that aligns with ADA Standards of Medical Care in Diabetes, AHA Hypertension Clinical Guidelines, and CDC Diabetes Prevention Program lifestyle recommendations.
• Work Monday through Friday for 7 hours each day, including a 1-hour lunch break, between 8 am and 8 pm EST.
• Establish a regular weekly schedule with the supervisor upon onboarding.
• Bachelor’s degree in Nursing, Nutrition, Health Promotion, Public Health, Health Education, Exercise Science, or a related field (required).
• National Board Certified Health and Wellness Coach (required).
• Registered Nurse or Registered Dietitian Nutritionist (preferred).
• Certified Diabetes Care and Education Specialist (preferred).
• CHES/MCHES certification (preferred).
• Strong understanding of HIPAA compliance.
• Minimum of 2 years of direct experience in diabetes care and education.
• At least 2 years of experience providing direct health education or health coaching focused on disease prevention, health promotion, and behavioral change.
• Familiarity with digital health technologies, coaching platforms, and Microsoft 365 (Word, Excel, Outlook, Teams).
• Knowledgeable about disease prevention and management, exercise principles, nutrition, and weight management, with and without GLP-1 medications.
• Capable of working independently as well as collaboratively within a team, demonstrating enthusiasm, initiative, and creativity.
• Flexible and able to juggle the demands of Diabetes coaching and Lifestyle coaching.
• Bilingual in English and Spanish is advantageous.
• Must work from a designated home office rather than a public location.
