Health Coach

This is a fully remote position, open to applicants in Arizona, +15 more states.

📋 Description

• Consistently utilize Vori Health's behavior change solutions.

• Apply coaching strategies to enhance patient outcomes.

• Conduct video consultations and messaging to support patients in achieving their wellness objectives.

• Create and maintain patient care plans employing motivational interviewing, SMART goals, coaching best practices, and behavior change methods.

• Oversee a significant patient caseload.

• Record interactions and interventions through Vori Health's technological tools.

• Collaborate with cross-functional teams and interdisciplinary healthcare professionals.

• Regularly engage with physicians, physical therapists, advanced practice providers, dietitians, and other specialists.

• Address issues, prioritize patient requirements, and prevent gaps in care.

• Align patient needs with suitable providers and assess referral urgency.

• Advocate for technological and service innovations to ensure closed-loop communication and care coordination.

• Provide and accept performance feedback.

• Communicate and collaborate effectively with external clinicians.

• Stay updated on professional and technical knowledge through workshops and published materials.

• Adhere to company and quality standards.

• Execute other assigned projects and responsibilities.

⛳️ Requirements

• Must be a Board Certified Health and Wellness Coach.

• Proficient in computers, virtual care platforms, database management, and data tracking systems.

• Familiarity with Motivational Interviewing, SMART goals, and behavioral stages of change.

• Minimum of 2 years of experience as a health coach or wellness educator conducting individual consultations or group workshops.

• Demonstrated competence and enthusiasm for digital care delivery or telehealth.

• Comfortable assessing and treating patients remotely using digital tools.

• Excellent written and verbal communication skills.

• Proven ability to match patient needs with treatment plans and execute them safely and effectively.

• Proficient in Microsoft Office Suite or similar software.

• Must have a reliable high-speed internet connection.

• Dedicated private office space free from distractions and noise.

• Capability to thrive in a fast-paced startup environment with ongoing changes.

• Exceptional time management skills and ability to meet deadlines.

• Authorized to work in the United States or able to provide necessary documentation.
