This is a fully remote position, open to applicants in Wisconsin.
• Schedule appointments for members regarding annual or change-based functional eligibility re-evaluations.
• Maximize time efficiency and travel resources.
• Utilize the State Department of Health Services screening tool to evaluate members' functional capabilities, health status, and necessary support.
• Work in collaboration with staff to ensure assessment data aligns with internal records.
• Conduct face-to-face interviews in members' homes.
• Input assessment data into the specified online system to ascertain the appropriate level of care.
• Timely submission of documentation in accordance with established protocols.
• Attend meetings and training sessions concerning processes, screening tools, and state regulations.
• Engage in quality assurance initiatives.
• Maintain required certification through continuous skills assessment.
• A four-year bachelor's degree in a field related to Health or Human Services, along with one (1) year of experience with at least one of the Family Care target populations, OR a Wisconsin licensed Registered Nurse with one (1) year of experience with at least one of the Family Care target populations.
• Compliance with Long Term Care Functional Screener (LTCFS) certification requirements; the selected candidate must complete online screener certification training courses and achieve certification as a functional screener.
• Possession of a valid state driver's license.
• Evidence of personal vehicle liability insurance with coverage limits of at least 100/300/100.
• Availability to work part-time Monday through Friday between 8:00 am and 4:30 pm CST.
• Willingness to travel up to 50% of the time.
• Required TB screening for this patient-facing position.
• A dedicated workspace that ensures the protection of member PHI/HIPAA information.
• Access to home internet with a minimum download speed of 25 Mbps and upload speed of 10 Mbps.
