Field Service Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• Evaluate and assess the needs and requirements of members to promote optimal wellness.

• Meet with members at their locations to identify needs, barriers, goals, and priorities.

• Perform initial and ongoing long-term services and support (LTSS) assessments utilizing person-centered thinking methodologies.

• Contact members through phone and/or in-person meetings.

• Create and continuously update service plans.

• Include care-team members in the care planning process.

• Identify LTSS providers and foster community partnerships.

• Assist members with LTSS, environmental, social, housing, educational, and additional services.

• Aid members in retaining Medicaid eligibility.

• Avoid redundant assessments by utilizing existing member information.

• Collaborate with Medical Directors, geriatricians, and care coordinators to ensure cohesive service delivery and positive outcomes.

⛳️ Requirements

• A bachelor's degree in a field related to health and human services.

• Two or more years of relevant experience.

• Proficiency in Microsoft Office Word and Excel.

• Outstanding verbal and written communication skills along with strong interpersonal abilities.

• Must live in Cass, Fulton, Howard, Miami, Tipton, or Wabash County, Indiana.

• Experience with electronic case notes and various computer applications/systems is preferred.

• Background in health promotion, coaching, and wellness is advantageous.

• Familiarity with community health and social service agencies as well as community resources is preferred.

• Access to home internet with a minimum download speed of 25 Mbps and upload speed of 10 Mbps.

• A dedicated workspace free from ongoing interruptions to safeguard member PHI / HIPAA information.
