Field Nurse Case Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Coordinate care and convey essential medical information among the Injured Worker, Insured, and Carrier.

• Facilitate suitable treatment, successful rehabilitation, and a return to work.

• Provide efficient, cost-effective communication regarding work injuries, MVAs, liability claims, LTD/STD, and other referred services.

• Adhere to all policies and procedures outlined in the Case Management plan and take accountability for the CM process.

• Accept referrals assigned by the Director of Case Management.

• Initiate contacts, schedule initial assessments, and acquire the next physician appointment date.

• Confirm assignments with referral sources and clarify any special handling instructions.

• Build and maintain professional relationships with clients and treat claimants with respect.

• Ensure ongoing communication with the Injured Worker, Insured, and Carrier.

• Document data and billing in CaseAnyplace.

• Submit timely monthly Preliminary, Initial, Progress, and Closing reports.

• Attend physician appointments to gather diagnosis, prognosis, treatment plans, rehabilitation duration, estimated return to work, and MMI/Full Recovery information.

• Refer to Vocational Counselors for job analyses when deemed appropriate and approved by the Carrier.

• Recommend and coordinate IME physicians and participate in IMEs.

• Arrange transportation as necessary.

• Provide translation services as needed for bilingual nurses.

• Monitor treatment plans, attend therapy sessions when appropriate, and maintain contact with therapists.

• Request file transfers to Vocational Counselors when suitable.

• Assist Carrier/Insured with modified or regular-duty return-to-work planning.

• Provide information to Defense Attorneys as necessary.

• Foster teamwork among all staff members.

• Maximize accurate, appropriate billable hours to meet the monthly target of 8 hours/day.

• Retain responsibility for tasks assigned to non-clinical staff.

• Undertake additional professional duties as assigned.


⛳️ Requirements

• Current, unrestricted Registered Nurse state license.

• Licensed in every state where field case management is conducted.

• Capable of performing independent assessments within the designated scope.

• Practices in a U.S. state/territory that permits independent assessment within the scope of practice.

• Completion of a nursing program and ongoing continuing education as required.

• One or more national certifications within 4 years of hire (e.g., CCM, CRC, CLNC, CRRN) preferred.

• Two years of full-time direct case management experience for injured workers or two years under supervision preferred.

• Maintain licensure/certifications.

• Complete required annual training punctually.
