Field Nurse Case Manager

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Coordinate care and share relevant medical information among the Injured Worker, Insured, and Carrier.

• Facilitate appropriate treatment, successful rehabilitation, and the return to work of injured individuals.

• Provide efficient and cost-effective communication regarding work injuries, MVAs, liability claims, LTD/STD, and other referred services.

• Adhere to policies and procedures outlined in the Case Management plan while remaining accountable for the CM process.

• Accept referrals designated by the Director of Case Management.

• Make initial contacts, schedule initial assessments, and gather upcoming physician appointment dates.

• Confirm assignments with referral sources and clarify any special handling instructions.

• Develop professional relationships with clients and ensure claimants are treated with respect and dignity.

• Maintain continuous communication with the Injured Worker, Insured, and Carrier.

• Document data and billing in CaseAnyplace.

• Submit timely monthly Preliminary, Initial, Progress, and Closing reports.

• Attend physician appointments to acquire diagnosis, prognosis, treatment plans, rehabilitation duration, estimated return-to-work status, and MMI/Full Recovery information.

• Refer to Vocational Counselors for job analyses as appropriate and as approved by the Carrier.

• Recommend and coordinate IME physicians and attend IMEs as needed.

• Arrange transportation when necessary.

• Monitor treatment plans and attend therapy sessions when appropriate.

• Maintain contact with therapists for updates on patient progress.

• Request file transfers to Vocational Counselors when appropriate.

• Assist Carrier/Insured with planning for modified or regular-duty returns to work.

• Provide information to Defense Attorneys when appropriate.

• Foster teamwork among all staff members.

• Maximize accurate and appropriate billable hours to meet the monthly target of 8 hours per day.

• Retain responsibility for tasks assigned to non-clinical staff.

• Undertake additional professional duties as assigned.


⛳️ Requirements

• Current, unrestricted Registered Nurse state license.

• Licensed in each state where field case management is conducted.

• Capable of performing independent assessments within the scope of practice.

• Practices in a U.S. state/territory that permits independent assessment within the defined scope.

• Completion of a nursing program and ongoing continuing education as required.

• One or more national certifications within four years of hire, such as CCM, CRC, CLNC, or CRRN (preferred).

• Two years of full-time equivalent direct case management experience for injured workers or two years of supervised experience (preferred).

• Ability to provide translation services as needed for bilingual nurses.

• Maintain licensure/certifications and complete required annual training on schedule.
