This is a fully remote position, open to applicants in Illinois.

📋 Description

• Coordinate care and relay essential medical information between the Injured Worker, Insured, and Carrier.

• Facilitate suitable treatment, effective rehabilitation, and a smooth return to work.

• Provide efficient and cost-effective communication regarding work injuries, MVAs, liability claims, LTD/STD, and other referred services.

• Adhere to the Case Management plan and maintain accountability for the Case Management process.

• Accept referrals as assigned by the Director of Case Management.

• Initiate contacts, arrange initial assessments, and secure physician appointment dates.

• Verify assignments with referral sources and clarify any special handling instructions.

• Cultivate professional relationships with clients and treat claimants with respect and dignity.

• Maintain continuous communication with the Injured Worker, Insured, and Carrier.

• Document data and billing in CaseAnyplace.

• Submit timely monthly Preliminary, Initial, Progress, and Closing reports.

• Attend physician appointments to gather diagnosis, prognosis, treatment plans, rehabilitation duration, estimated return-to-work status, and MMI/Full Recovery details.

• Refer to Vocational Counselors for job analyses when deemed appropriate and approved by the Carrier.

• Recommend and coordinate Independent Medical Examinations, and attend IMEs.

• Organize transportation as necessary.

• Provide translation services as needed for bilingual nurses.

• Monitor treatment plans and participate in therapy sessions when necessary.

• Maintain communication with therapists for updates.

• Request file transfers to Vocational Counselors when appropriate.

• Assist the Carrier/Insured with planning for modified or regular-duty return-to-work.

• Provide relevant information to Defense Attorneys as needed.

• Foster teamwork among all staff members.

• Optimize accurate, appropriate billable hours toward the monthly target of 8 hours per day.

• Retain responsibility for tasks delegated to non-clinical staff.

• Carry out additional professional duties as assigned.


⛳️ Requirements

• Current, unrestricted Registered Nurse state license.

• Licensed in each state where field case management services are provided.

• Capable of conducting independent assessments within scope.

• Practices in a U.S. state/territory that permits independent assessments within the scope of practice.

• Completion of an accredited nursing program.

• Ongoing continuing education as mandated.

• One or more national certifications within 4 years of hire (e.g., CCM, CRC, CLNC, CRRN) preferred.

• A minimum of two years FTE direct case management experience for injured workers or two years under supervision preferred.

• Maintain licensure and certifications.

• Complete required annual training punctually.
