Field Community Health Worker, Wayne or Macomb County

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Act as a key member of care teams and interact directly with members.

• Assist with case management duties and address social determinants of health needs.

• Aid members in managing their chronic conditions independently.

• Navigate healthcare systems while advocating for prevention and health education.

• Carry out in-person evaluations of care needs, preferences, socioeconomic challenges, and living situations.

• Facilitate access to housing, food, income, transportation, healthcare, and social services.

• Arrange appointments for physical and behavioral health visits.

• Serve as an advocate for members with healthcare providers, community resources, educational institutions, and others.

• Recognize and tackle obstacles to healthy living and access to healthcare.

• Provide support for high-risk and hard-to-reach member groups.

• Encourage adherence to care plans through motivational interviewing techniques.

• Deliver social support and culturally suitable health education and coaching.

• Conduct research and outreach to connect with hard-to-reach members.

• Foster relationships with providers and community resources while identifying service gaps.

• Travel frequently for visits to members, providers, and community events.

• Collaborate with care coordinators, housing specialists, and SDOH coordinators.

• Participate in community events to engage with members and educate them about case management services.

⛳️ Requirements

• Must live in Michigan and be able to travel within Wayne or Macomb Counties.

• At least 2 years of experience working with community resources, health agencies, or social services.

• Intermediate skills in Microsoft Office applications (Teams, Excel, PowerPoint, Outlook, Word).

• Commitment to respecting cultural and demographic diversity.

• Strong written communication abilities.

• Advocate for members across all levels of care.

• Oral and written fluency in English, Arabic, and/or Chaldean Neo-Aramaic; language assessment required.

• Valid driver's license from the state.

• Personal vehicle liability insurance that meets state minimum requirements or specified coverage limits.

• Must pass TB screening.

• Availability to work Monday through Friday, from 8:00 AM to 5:00 PM Eastern Time.

• Community Health Worker training and/or certification is preferred.

• A Bachelor’s Degree in Social Work or a related field is preferred.

• Familiarity with state Medicaid program guidelines is preferred.

• Experience working with Medicaid enrollees is preferred.
