Field Care Manager, Nurse – BSN

This is a fully remote position, open to applicants in South Carolina.

📋 Description

• Evaluate and assess the needs and requirements of members to achieve or sustain optimal wellness.

• Guide members and their families toward suitable care and wellbeing resources.

• Facilitate connections with care and community resources.

• Manage the physical, environmental, and psychosocial health issues of members through various strategies and techniques.

• Identify and address barriers to effective care.

• Monitor patient care through continual assessments and evaluations.

• Ensure that patients make progress toward desired health outcomes.

• Develop member care plans as necessary.

• Implement departmental, segmental, and organizational strategies and operational objectives.

• Travel up to 50% to foster collaboration, conduct in-person meetings, and engage with staff, providers, members, and families.

• Report to the Manager of Care Management for Behavioral Health.

⛳️ Requirements

• Bachelor’s degree in nursing (BSN).

• Hold an active license in the state of South Carolina with no disciplinary actions.

• Must reside in South Carolina.

• At least 2 years of experience in case or care management.

• Minimum of 2 years working with the behavioral health population.

• Familiarity with community health and social service agencies, as well as additional community resources.

• Proficient in using electronic information applications and software programs, including electronic medical records.

• Reside near the Midlands or Upstate area corresponding to your region.

• Have a dedicated workspace free from ongoing interruptions to safeguard member PHI/HIPAA information.

• Provide your own internet service with a minimum of 25 Mbps download and 10 Mbps upload speed.

• Preferred: 3 or more years of experience in in-home assessment and care coordination.

• Preferred: Experience in health promotion, coaching, and wellness.

• Preferred: Previous experience in managed care.

• Preferred: Bilingual in English and Spanish.

• Preferred: Certification in Case Management.

• Preferred: Certification in Motivational Interviewing or relevant knowledge.
