Field-Based Home Care Consultant (m/f/d)

This is a fully remote position, open to applicants in Germany.

📋 Description

• Conduct home-based consultation sessions with insured individuals and their family members in accordance with Section 37(3) of Book XI of the German Social Code (SGB XI).

• Document consultation sessions, assess individual needs, and initiate suitable follow-up actions.

• Provide training for family caregivers in line with Section 45 of SGB XI.

• Offer advice to individuals in need of care and their relatives as per Section 7a of SGB XI.

• Deliver product training and offer professional guidance to employees across various companies.

• Assist in the design and creation of informational materials.

• Establish and enhance regional networks and partnerships.

⛳️ Requirements

• Completed training as a healthcare and nursing professional, geriatric nurse, registered nurse, or a similar qualification.

• A minimum of two years of professional experience.

• At least six months of experience specifically in home healthcare.

• Qualifications related to Sections 45 and/or 7a of SGB XI, or a willingness to acquire them.

• Proficient in using MS Office and contemporary digital work tools.

• Capability to share knowledge and provide support to individuals.

• Understanding of the emotional significance of home and its role in caregiving.

• Strong communication skills to assist individuals in balancing work and caregiving responsibilities.

• Willingness to travel within the region.

• A Class B driver's license.
