Evening Home Care & Hospice Virtual Care Coordinator, RN

This is a fully remote position, open to applicants in Florida.

📋 Description

• Conduct virtual evaluations and monitor the conditions of patients from a distance.

• Create and coordinate personalized care plans in collaboration with the interdisciplinary team.

• Offer virtual education and assistance to patients, families, and caregivers concerning symptom management and end-of-life care.

• Collaborate with field nurses, physicians, social workers, and other team members to guarantee comprehensive care delivery.

• Quickly respond to urgent virtual consultations and assess patient needs.

• Utilize telehealth technology for regular check-ins and follow-ups with patients and their families.

⛳️ Requirements

• Over 5 years of experience in Home Care, Hospice, ER, and/or Triage as a Registered Nurse.

• A valid RN license in a Compact State (eNLC).

• A Bachelor of Science in Nursing (BSN) from an accredited nursing education program.

• Familiarity with Epic or experience with Home Care EMR systems.

• Excellent communication and interpersonal skills to establish rapport with patients and families virtually.

• Capability to work independently as well as part of a collaborative remote team.

• Flexibility to adapt to rapid changes to fulfill business and departmental requirements.

• Experience in patient or customer service.

• Availability to work at least 30 hours within a six-week timeframe, dependent on business needs.

• Willingness to work from 2:00 p.m. to 12:00 a.m. EST.
