This is a fully remote position, open to applicants in Washington.
• Facilitate the completion of CHE in nephrology offices, dialysis clinics, or through telehealth.
• Collaborate with Nurse Practitioners and clinics to prepare for in-person appointments.
• Conduct outbound calls for scheduling CHEs, sending reminders, and rescheduling appointments.
• Arrange telehealth CHEs for patients who are unable to attend in person.
• Welcome new patients and gather concerns from existing patients; escalate issues to RN or Nurse Practitioner as necessary.
• Ensure accurate documentation and communicate patient updates to the care team.
• Foster trust, encourage patient participation, and navigate engagement challenges.
• Organize outreach efforts based on urgency and importance.
• Coordinate care by ensuring access to eligible resources and assisting with referrals and provider appointments.
• Engage in outbound and inbound calls with patients, providers, and Village resources.
• Complete non-clinical screenings and pathways under the guidance of the Manager; escalate issues as needed.
• Support the clinical team with continuum-of-care tasks.
• Identify eligible resources and work with nurses, clinics, health plans, and stakeholders to enroll members.
• Communicate with patients promptly and professionally.
• Maintain accurate, complete, and confidential documentation.
• Provide high-quality patient care in alignment with DaVita’s mission and values.
• High School Diploma or GED is required.
• Previous experience in IKC is necessary.
• A minimum of 2 years of professional experience in customer service, patient-facing, or telephonic outreach roles is preferred.
• Prior experience in the healthcare field is preferred.
• Intermediate proficiency in Microsoft Word, Excel, PowerPoint, and Outlook is essential.
• Strong verbal and written communication abilities are required.
• Skills in active listening and motivational engagement techniques are important.
• Highly organized, with excellent time management and follow-through capabilities.
• Ability to work both independently and collaboratively with care team members.
• A quiet, private home workspace with reliable high-speed internet is necessary.
