This is a fully remote position, open to applicants in California.
• Oversee, coordinate, and manage the day-to-day operations of ECM and CICM staff.
• Directly supervise Care Managers, Case Managers, Community Health Workers, and other designated personnel.
• Conduct individual supervision sessions, team meetings, and case consultations.
• Offer coaching, mentoring, and opportunities for professional development.
• Engage in recruitment, orientation, training, and onboarding processes.
• Monitor staff productivity, performance objectives, quality metrics, and utilization statistics.
• Execute performance evaluations and assist in developing corrective action plans.
• Manage daily ECM/CICM operations, workflow, member assignments, and caseload distribution.
• Oversee member engagement, outreach initiatives, and service delivery.
• Identify operational challenges and implement process enhancements.
• Provide guidance on complex member needs and high-risk cases.
• Ensure timely completion of assessments, care plans, care coordination activities, and related documentation.
• Review high-risk, escalated, or complex cases and participate in interdisciplinary case conferences.
• Promote person-centered care planning and holistic care principles.
• Facilitate collaboration with medical, behavioral health, social services, community-based organizations, and long-term care providers.
• Conduct chart reviews and audits of documentation.
• Ensure compliance with ECM/CICM requirements, health plan expectations, and DHCS regulations.
• Track quality indicators and corrective action efforts.
• Assist with internal and external audits and initiatives for quality improvement.
• Support the implementation of performance improvement plans.
• Maintain relationships with managed care organizations and community partners.
• Participate in health plan meetings, case conferences, and collaborative efforts.
• Perform additional duties as assigned.
• A Bachelor's degree in Social Work, Nursing, Healthcare Administration, Human Services, Public Health, or a related field is required.
• A Master's degree is preferred.
• At least 3 years of experience in case management, care management, social services, population health, or healthcare operations.
• A minimum of 1-2 years of supervisory or team leadership experience.
• Experience with Medi-Cal populations and complex care management programs is essential.
• Preferred experience with ECM, CalAIM, CICM, care coordination, or managed care programs.
• Capability to supervise Care Managers, Case Managers, Community Health Workers, and other assigned staff.
• Ability to monitor productivity, performance objectives, quality metrics, compliance, and utilization statistics.
• Proficiency in conducting chart reviews, documentation audits, performance evaluations, and corrective action plans.
• Ability to manage assessments, care plans, care coordination activities, encounters, member interactions, and documentation.
• Competence in facilitating interdisciplinary collaboration with medical, behavioral health, social service, community-based, and long-term care providers.
• Ability to support audits, quality improvement initiatives, and performance improvement plans.
• Frequent use of computer, telephone, and keyboard is required.
• Ability to drive up to 50% of the time.
