ECM/CICM Supervisor

This is a fully remote position, open to applicants in California.

📋 Description

• Directly oversee Care Managers, Case Managers, Community Health Workers, and other designated personnel.

• Facilitate regular one-on-one supervision sessions, team meetings, and case consultations.

• Offer coaching, mentoring, and professional growth opportunities.

• Engage in the recruitment, orientation, training, and onboarding processes for new employees.

• Track staff productivity, performance objectives, and quality indicators.

• Conduct performance assessments and aid in the development of corrective action plans.

• Manage daily ECM/CICM operations and workflow processes.

• Appropriately assign and distribute members and caseloads.

• Monitor member engagement, outreach efforts, and service provision.

• Identify operational challenges and execute process enhancements.

• Provide support for complex member requirements and high-risk situations.

• Ensure timely execution of assessments, care plans, care coordination tasks, and documentation.

• Review high-risk, escalated, or intricate cases and take part in interdisciplinary case discussions.

• Advocate for person-centered care planning and holistic care principles.

• Collaborate with medical, behavioral health, social service, community-based, and long-term care providers.

• Conduct chart reviews and audits of documentation.

• Ensure compliance with ECM/CICM standards, health plan requirements, and DHCS regulations.

• Monitor quality metrics and corrective action initiatives.

• Assist with both internal and external audits as well as quality improvement projects.

• Supervise productivity and utilization metrics.

• Support performance enhancement plans.

• Maintain partnerships with managed care organizations and community collaborators.

• Participate in health plan meetings, case discussions, and collaborative efforts.

• Execute additional duties as assigned.

⛳️ Requirements

• A Bachelor's degree in Social Work, Nursing, Healthcare Administration, Human Services, Public Health, or a related discipline is required.

• A Master's degree is preferred.

• At least 3 years of experience in case management, care management, social services, population health, or healthcare operations.

• A minimum of 1-2 years of experience in a supervisory or team leadership role.

• Experience with Medi-Cal populations and complex care management programs is essential.

• Familiarity with ECM, CalAIM, CICM, care coordination, or managed care programs is preferred.

• Capability to stand, sit, talk, hear, and use hands and fingers to operate a computer, telephone, and keyboard frequently, up to 20% of the time.

• Ability to reach, stoop, kneel, and bend up to 15% of the time.

• Moderate walking is required up to 15% of the time.

• Moderate driving is necessary up to 50% of the time.

• Close vision is needed for computer-related work.

• Ability to frequently lift up to 25 lbs; push and pull up to 25 lbs.
