ECM Care Manager – Housing Specialist

This is a fully remote position, open to applicants in California.

📋 Description

• Oversee a designated caseload of ECM and CS members.

• Facilitate healthcare services, referrals, and access to community resources.

• Provide assistance with housing searches, applications, necessary documentation, and ensuring housing stability.

• Engage with healthcare providers, landlords, property managers, and community partners.

• Conduct in-person appointments, telehealth consultations, and follow-up outreach efforts.

• Keep accurate and timely contact notes, care plans, and housing plans.

• Participate in a scheduled monthly outreach week, typically occurring at the beginning or end of the month.

• Organize field visits based on location.

• Relay member needs or obstacles to the supervisor.

• Assist members receiving Enhanced Care Management and Community Supports in obtaining healthcare, community resources, and stable housing.

⛳️ Requirements

• Must be a resident of California.

• Willingness to travel regularly across Monterey, San Benito, Mariposa, and Contra Costa counties.

• Possession of a valid driver’s license and reliable transportation.

• Proficient in computer skills, communication, organization, and documentation.

• Capability to work independently while ensuring member confidentiality.

• Preferred experience in case management, housing support, social services, or community outreach.

• A real estate license is not necessary.

• Availability for one weekly shift from 7 AM to 3 PM, three shifts from 9 AM to 5 PM, and one shift from 10 AM to 6 PM.

• Bilingual proficiency in Spanish is included in the application screening questions.
