This is a fully remote position, open to applicants in Tennessee.
• Empower individuals by assisting them in achieving their health and wellness goals.
• Support members in securing and sustaining competitive, integrated employment opportunities.
• Navigate complex relationships to ensure members receive essential services.
• Conduct intake, enrollment, and ongoing evaluations.
• Create and sustain Person-Centered Plans.
• Oversee and ensure comprehensive service delivery.
• Collaborate with members and their families to identify support and direct services for health, employment, and independent or community living objectives.
• Work alongside clinical and social support teams to address physical, behavioral health, and long-term service requirements.
• Perform thorough, objective face-to-face assessments to evaluate members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs.
• Apply criteria to authorize suitable home and community-based services, ensuring that services are delivered and members' needs are satisfied.
• RN; or a Bachelor’s degree in social work, nursing, education, human services, health care profession, or a related field; or fulfillment of federal criteria for a Qualified Developmental (or Intellectual) Disabilities Professional (QDDP/QIDP).
• RN candidates should ideally possess current Developmental Disabilities Nurses Association certification as CDDN; LPNs as DDC.
• Preference for Licensed Master Social Worker or Licensed Clinical Social Worker credentials.
• In the absence of a degree, a minimum of 5 years’ experience as an independent Support Coordinator or case manager for service recipients in a 1915(c) HCBS Waiver, along with completed Personal Outcome Measures Introduction and Assessment Workshop trainings.
• At least 2 years’ experience in the healthcare sector or a similar operational environment is required.
• A valid driver's license is necessary.
• Various immunizations and/or related medical evaluations may be mandated.
• A digital literacy assessment is required.
• Proficiency in Microsoft Office (Outlook, Word, Excel, and PowerPoint) is essential.
• Capability to work independently with minimal oversight or within a team setting.
• Strong problem-solving skills and the ability to manage multiple tasks with critical deadlines.
• Exceptional oral and written communication abilities.
• Strong interpersonal and organizational competencies.
• A valid driver’s license, compliant auto insurance, and an acceptable motor vehicle record are required for regular company driving.
• Mileage reimbursement for travel to members’ residences.
• A schedule from 8:00am to 5:00pm with no on-call obligations.
