EAP Care Advocate, Licensed Counselor/Social Worker

This is a fully remote position, open to applicants in United States.

📋 Description

• Offer guidance, support, consultation, and referrals to eligible employees and their dependents.

• Perform thorough clinical assessments, including identifying presenting issues, work-life challenges, and screening for risk and safety concerns.

• Create personalized care plans that cater to each participant's distinct clinical and sub-clinical needs.

• Coordinate referrals and assist participants in securing appointments for short-term EAP counseling.

• Provide mental health advocacy and navigation for participants requiring higher levels of care.

• Help with referrals to other employer-sponsored health management resources when suitable.

• Engage in active case management, including reviewing treatment plans.

• Conduct prompt follow-ups to ensure connection to care, program satisfaction, and assess EAP service outcomes and impact.

• Keep accurate and complete records in adherence to HIPAA, ethical, and departmental standards.

⛳️ Requirements

• Master's Degree in a Mental Health Discipline (e.g., Social Work, Counseling, Marriage and Family Therapy).

• Relevant Post-Graduate Clinical Experience.

• Clinical Licensure.

• Proficiency and comfort with technology for remote training and work (e.g., Microsoft Office Suite, Contact Management Systems, Web-Based Software, Soft Phone).

• Strong analytical skills – Ability to collect data, establish facts, define problems, and draw valid conclusions.

• Capability to excel in a fast-paced, fully remote work environment.

• Must be located in the continental United States or Hawaii.

• Previous experience in EAP and/or Case Management is advantageous.
