Diversion Navigator

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Perform initial screenings, crisis stabilization, and diversion assessments for individuals seeking emergency shelter.

• Partner with clients to create diversion plans that may include landlord or family mediation, short-term financial aid, or other housing resources.

• Foster relationships with community partners to ensure clients receive suitable services.

• Collaborate with County Financial Assistance staff to confirm eligibility, finalize documentation, and facilitate payments for rent, utilities, or damage deposits when necessary.

• Accurately and promptly track and report required data in HMIS and other systems.

• Maintain an up-to-date resource referral database.

• Keep informed about community programs, coordinated assessment processes, and housing stabilization efforts.

• Offer education, coaching, and advocacy to assist clients in navigating systems and attaining housing stability.

• Contribute to program evaluation, enhance service delivery, and participate in collaborative team meetings.

• Represent Catholic Charities at designated community events and meetings.

• Execute other assigned responsibilities.

⛳️ Requirements

• A post-secondary education is required.

• A minimum of two (2) years of relevant experience in human services, housing, or related fields.

• Familiarity with housing programs and municipal regulations, including landlord/tenant laws and foreclosure processes.

• Experience in conducting intake, assessment, or eligibility determination procedures.

• Background in landlord/tenant mediation, conflict resolution, or housing navigation.

• Working knowledge of community resources available for individuals and families facing homelessness or housing instability.

• Proven ability in relationship-building and effective interpersonal communication, both written and verbal.

• Strong computer skills, including proficiency with HMIS and Microsoft Office applications.

• Capability to work effectively with diverse and low-income populations, including those experiencing homelessness, unstable housing situations, or mental health challenges.
