This is a fully remote position, open to applicants in North Carolina.
β’ Collaborate with local facility leaders, clinical teams, operational stakeholders, and enterprise Community Health & Impact leaders to synchronize community health priorities with organizational strategies and population health objectives.
β’ Direct the Community Health Needs Assessment cycle along with associated Implementation Strategies and governance duties.
β’ Create, execute, and enhance community health programs that tackle social determinants of health.
β’ Set performance metrics, evaluation methodologies, and reporting systems.
β’ Identify and evaluate local community projects for potential low-interest loan investments.
β’ Assist in Community Engagement strategies and foster relationships with community-based organizations, public agencies, civic leaders, and philanthropic collaborators.
β’ Supervise the entry, documentation, and validation of community benefit data.
β’ Convert community data, partner insights, and implementation progress into actionable recommendations, presentations, and updates.
β’ Assemble internal and external partners, track progress, and address barriers to implementation.
β’ A Bachelor's degree in Public Health, Public Administration, Community Planning, Health Administration, or a related field.
β’ A minimum of 7 years of experience in addressing and enhancing the social determinants of health, community health strategy, population health initiatives, or similar fields.
β’ Proven experience in developing strategies aimed at improving health in underserved communities.
β’ Capability to foster clarity, alignment, and decision-making support throughout a complex organization, including engagement with senior executives.
β’ Familiarity with current and evolving best practices in large health systems concerning community needs, social determinants of health, and health disparities.
β’ Understanding of public health data collection and analytical techniques.
β’ Acquainted with Community Benefit Reporting methodologies and standards.
β’ Experience in tackling social determinants of health in collaboration with coalitions and community-based partners.
β’ Strong analytical and writing capabilities.
β’ Ability to build, convene, and align cross-functional teams and working groups towards common objectives.
β’ Proficient in developing and managing projects within intricate community settings.
β’ Competence in operating necessary equipment to perform job functions.
β’ Capability to work from a location outside of Advocate Health.
β’ A Master's degree is preferred.
