This is a fully remote position, open to applicants in Illinois.
β’ Collaborate with local facility leaders, clinical teams, operational stakeholders, and enterprise Community Health & Impact leaders to align community health priorities with the organization's strategies and population health objectives.
β’ Direct the Community Health Needs Assessment cycle, Implementation Strategies, and governance duties for designated markets or facilities.
β’ Create, execute, and enhance community health programs that address key social determinants of health.
β’ Set performance metrics, evaluation methods, and reporting procedures to assess program efficacy and support ongoing improvement.
β’ Identify, evaluate, and promote local community projects for potential low-interest loan investment.
β’ Assist in Community Engagement strategies and foster relationships with community-based organizations, public agencies, civic leaders, philanthropic partners, and local stakeholders.
β’ Manage community benefit data entry, documentation, and verification processes.
β’ Interpret community data, partner insights, and implementation progress into actionable recommendations, presentations, and updates.
β’ Bring together internal and external partners, track progress against objectives, and address implementation challenges.
β’ Bachelorβs degree in Public Health, Public Administration, Community Planning, Health Administration, or a related field.
β’ 7 years of experience in addressing and enhancing the social determinants of health, community health strategy, population health initiatives, or similar areas.
β’ Proven experience in formulating strategies to improve health outcomes in underserved communities.
β’ Ability to foster clarity, alignment, and decision-making support across a complex organization, including engagement with senior executives.
β’ Familiarity with current and emerging best practices in large health systems that address community needs, social determinants of health, and health disparities.
β’ Understanding of public health data collection and analysis.
β’ Knowledge of Community Benefit Reporting methods and standards.
β’ Experience in working with coalitions and community-based partners to address social determinants of health.
β’ Strong analytical and writing capabilities.
β’ Proven ability to build, convene, and align cross-functional teams and working groups towards common goals.
β’ Capacity to lead through influence and ensure accountability within a matrixed organization.
β’ Ability to develop and manage projects in complex community environments.
β’ Willingness to travel locally and, on occasion, enterprise-wide.
β’ Proficiency in operating necessary equipment.
β’ Capability to work virtually from a non-Advocate Health location.
