This is a fully remote position, open to applicants in United States.
β’ Collaborate with local facility leaders, clinical teams, operational stakeholders, and enterprise Community Health & Impact leaders to align community health priorities with organizational strategies, service line objectives, and population health targets.
β’ Direct the Community Health Needs Assessment cycle, associated Implementation Strategies, and governance responsibilities for designated markets or facilities.
β’ Design, implement, and enhance community health programs that address priority social determinants of health and improve both social and clinical outcomes.
β’ Establish performance metrics, evaluation methods, and reporting systems to measure program effectiveness and guide ongoing improvements.
β’ Identify, evaluate, and promote local community initiatives for potential low-interest loan investments.
β’ Support Community Engagement strategies and foster relationships with community-based organizations, public entities, civic leaders, philanthropic partners, and local stakeholders.
β’ Supervise community benefit data entry, documentation, and validation processes.
β’ Interpret community data, partner insights, and implementation progress to create recommendations, presentations, and updates.
β’ Gather internal and external partners, track progress towards goals, and tackle implementation challenges.
β’ Travel locally within the assigned service area and occasionally across enterprise service areas.
β’ A Bachelor's degree in Public Health, Public Administration, Community Planning, Health Administration, or a related field.
β’ At least 7 years of experience in addressing and enhancing social determinants of health, community health strategy, population health initiatives, or similar areas.
β’ Proven experience in formulating strategies to improve health outcomes in underserved communities.
β’ Ability to foster clarity, alignment, and decision-making support throughout a complex organization, including senior executives.
β’ Understanding of current and emerging best practices in large health systems that address community needs, social determinants of health, and health disparities.
β’ Proficient in public health data collection and analysis.
β’ Familiarity with Community Benefit Reporting methods and standards.
β’ Experience in addressing social determinants of health in collaboration with coalitions and community-based partners.
β’ Strong analytical and writing abilities.
β’ Demonstrated capacity to build, convene, and unify cross-functional teams and working groups towards collective objectives.
β’ Ability to lead through influence and ensure accountability within a matrixed organization.
β’ Skilled in developing and managing projects within complex community settings.
β’ Capability to work from a non-Advocate Health location.
β’ Preferred: Master's degree in Public Health, Public Administration, Community Planning, Health Administration, or a relevant field.
β’ Preferred: experience in developing strategies, partnership approaches, or investment opportunities across various sectors and stakeholders.
β’ Preferred: demonstrated program/project management skills, including translating strategy into actionable workplans, managing timelines and milestones, coordinating cross-functional stakeholders, and providing clear status updates.
