This is a fully remote position, open to applicants in United States.
• Collaborate with local facility leaders, clinical teams, operational stakeholders, and enterprise Community Health & Impact leaders to synchronize community health priorities with organizational strategies and population health objectives.
• Direct Community Health Needs Assessment cycles, Implementation Strategies, and governance responsibilities for designated markets or facilities.
• Design, execute, and enhance community health programs that tackle priority social determinants of health.
• Establish performance metrics, evaluation methods, and reporting systems to gauge program efficacy and facilitate ongoing improvement.
• Identify, evaluate, and promote local community projects for potential low-interest loan investment.
• Assist in Community Engagement strategies and foster relationships with community organizations, public agencies, civic leaders, philanthropic partners, and local stakeholders.
• Supervise community benefit data entry, documentation, and validation processes.
• Convert community data, partner insights, and implementation progress into actionable recommendations, presentations, and updates.
• Assemble internal and external partners, track progress against objectives, and resolve implementation challenges.
• Travel locally within the assigned service area and occasionally across enterprise service areas.
• Maintain a work schedule of Monday to Friday, 8am to 5pm, with flexibility for weekends as necessary.
• A Bachelor's degree in Public Health, Public Administration, Community Planning, Health Administration, or another relevant discipline.
• A minimum of 7 years of experience in addressing and enhancing social determinants of health, community health strategy, population health initiatives, or related fields.
• Proven experience in developing strategies to improve health outcomes in underserved communities.
• Capability to drive clarity, alignment, and decision-making support throughout a complex organization, including interactions with senior-level executives.
• In-depth knowledge of current and emerging best practices in large health systems addressing community needs, social determinants of health, and health disparities.
• Familiarity with public health data collection and analysis methodologies.
• Understanding of Community Benefit Reporting approaches and standards.
• Experience in addressing social determinants of health through coalitions and partnerships with community-based organizations.
• Strong analytical and written communication skills.
• Ability to build, convene, and align cross-functional teams and working groups towards common objectives, lead through influence, and ensure accountability across a matrixed organization.
• Competency in developing and managing projects within complex community contexts.
• Proficiency in operating necessary equipment to perform job functions.
• Capability to work from a non-Advocate Health location.
• A Master's degree in a relevant field is preferred.
