This is a fully remote position, open to applicants in Canada.
• Oversee pediatric, palliative, and/or medically complex patients with COPD or CHF.
• Supervise Personal Support Workers/Health Care Technicians in delivering client care.
• Conduct remote assessments and manage care services in clients' residences.
• Provide clinical guidance and delegate tasks within PSW/HCT practice guidelines.
• Keep precise digital and paper clinical documentation.
• Manage patient assignments within the virtual ward.
• Collaborate with team members to address patient care requirements.
• Engage in daily huddles and respond to emails and phone inquiries.
• Liaise with external service provider agencies and Case Managers.
• Act as a member of the multidisciplinary healthcare team.
• Deliver clinical leadership, guidance, and mentorship.
• Foster a cooperative team-based atmosphere.
• Build and nurture relationships with local healthcare providers.
• Participate in CNO Reflective Practice.
• Adhere to VON standards and eHomecare protocols.
• Engage in internal projects and initiatives for continuous improvement.
• Exhibit professionalism and adaptability in a dynamic virtual setting.
• Registered and in good standing with the College of Nurses of Ontario.
• CNA Certificate in Palliative Care, or completion of both “Fundamentals of Palliative Care” and “Comprehensive Advanced Palliative Care Education (CAPCE).”
• Pediatric DRNs must possess extensive hands-on experience in Pediatric Care.
• Recent experience in palliative, pediatric, and/or acute medical care with COPD and CHF patients.
• Proven advanced assessment abilities.
• Experience in gerontology and/or medical-surgical nursing.
• Community-based healthcare experience is advantageous.
• Proficiency in observation, independent assessment, health education and promotion, and COPD/CHF clinical pathways.
• Familiarity with digital and electronic health, EMR, and mobile technology.
• Strong nursing skills and capability to interpret electronic technical information related to client outcomes.
• Critical thinking, problem-solving, and multitasking abilities.
• Capacity to work within a multidisciplinary virtual team and independently.
• High-speed internet, computer proficiency, and capacity to work from home in a private office setting.
• Eligible to successfully acquire a Government of Canada security screening certificate.
• Valid CPR C / First Aid certification.
• Personal vehicle, current driver’s license, and proof of vehicle insurance.
• A satisfactory Criminal Records Check is required upon commencing employment.
• Vulnerable Sector Search and/or Child Abuse Registry Check may be necessary.
• Proficiency in French is an asset in designated French areas.
• Capability to wear Personal Protective Equipment (PPE).
• Must fulfill the physical demands of lifting, carrying, bending, reaching, kneeling, walking, sitting, standing, climbing stairs, and fine hand movements.
