Connected Care Coach

This is a fully remote position, open to applicants in United States.

📋 Description

• Proactively reach out to designated members to encourage behavior modification and enhance clinical results.

• Interact with members through telephone, email, text, virtual applications, and coaching discussions.

• Offer feedback and assistance, respond to inquiries, share resources, and ensure members are accountable for their objectives.

• Employ a coaching strategy aligned with motivational interviewing to enable members to self-manage chronic illnesses and overall health.

• Refer members for condition-specific assistance and educational resources in accordance with clinical care protocols.

• Keep precise and comprehensive case records in line with Teladoc Health standards.

• Carry out administrative tasks necessary for efficient operations.

• Assist with corporate initiatives and special projects as required.

⛳️ Requirements

• Bachelor’s Degree in a relevant field or certification as an MA/EMT/Paramedic.

• A minimum of 2 years of experience in healthcare delivery or a related field.

• Excellent written and verbal communication abilities.

• Outstanding time management and organizational skills.

• Strong analytical and critical thinking capabilities.

• Capacity to work independently in a responsible and efficient manner.

• Exceptional customer service and interpersonal skills.

• Willingness to adapt in a fast-paced environment.

• Openness to teamwork and embracing change.

• Proficiency in utilizing multiple technology platforms, including MS Word, Excel, PowerPoint, Outlook 365, Microsoft Teams, SharePoint, OneDrive, Salesforce, and Jira.

• Bilingual in Spanish and English is preferred.

• Must be authorized to work in the United States without requiring visa sponsorship now or in the future.
