Complex Care Social Work Case Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Foster trust and cultivate robust relationships with patients through phone and virtual interactions.

• Work in partnership with clinicians, case managers, community health workers, and coordinators to deliver integrated, patient-focused care.

• Recognize social barriers, perform screenings, and develop tailored action plans.

• Identify signs of caregiver burnout and offer emotional support and resources.

• Address complex family and support dynamics.

• Guide discussions regarding end-of-life planning.

• Assess long-term care planning requirements and objectives, provide relevant information, arrange referrals, and coordinate care.

• Ensure seamless care coordination between the Devoted Medical Behavioral Health Team and track outcomes.

• Manage multiple competing priorities for complex patient cases.

• Establish connections and collaborate with local market and internal stakeholders.

• Capability to thrive in a dynamic, fast-paced environment.

• Willingness to embrace and adapt to evolving technologies.

• A minimum of 3 years of experience in medical social work, case management, or care coordination.

• Master’s degree in social work along with an active state license.

• Bilingual in Spanish is preferred.

• Experience in health insurance is preferred, with Medicare Advantage being a plus.

• Previous experience working with the senior demographic is advantageous.

• Demonstrated success in establishing relationships with patients and an interdisciplinary team.

• Proficiency in multitasking: listening, communicating, evaluating, and documenting simultaneously.

• Comfort with remote work settings.
