Community Resource Coordinator II

This is a fully remote position, open to applicants in Kansas.

📋 Description

• Facilitate community connection initiatives by linking members to community resources throughout their care management journey.

• Provide members with recognized community resources and assist the care team in identifying community support options.

• Coordinate outreach resources for the community and enhance awareness of care/services available.

• Assist members with inquiries regarding community and care resources.

• Prepare and support correspondence and educational materials for members.

• Document and maintain community resources according to health plan standards.

• Aid clinical nurses and social workers by providing outreach, community education, informal guidance, and member support.

• Conduct non-clinical general health assessments and refer members to suitable care/services.

• Perform non-medical assessments related to home safety, community/environment resources, transportation, employment, and other related needs.

• Engage in telephonic and/or in-person outreach to reach hard-to-access individuals and families.

• Visit individual homes and/or community organizations as required.

• Carry out additional duties as assigned.

⛳️ Requirements

• Must be a resident of the state of Kansas.

• Willingness to travel within Shawnee, Osage, Douglas, Anderson, Franklin, Butler, Chase, Lyon, Greenwood, and Coffey counties.

• High School diploma or GED required.

• 1 – 2 years of relevant experience needed.

• Familiarity with Social Determinants of Health (SDOH) barriers is essential.
